Since 2017, Alenta Service has helped Pennsylvanians and their families navigate the state's waiver programs — so people can live at home and in their community with the supports they need. Service coordination is free to eligible participants; it's funded by the waiver itself.
What is a service coordinator?
A service coordinator is your single point of contact for a Pennsylvania waiver. We assess needs, build a written service plan with you, connect you with approved providers, and monitor the plan over time — adjusting as your life changes. We do the paperwork, the phone calls, and the follow-through so you can focus on living.
Not eligible for a waiver? Families paying privately for care can explore our private geriatric care management service instead.

Aging Waiver
Long-term care services for qualified older Pennsylvanians who choose to live at home and in their community instead of a nursing facility.
Attendant Care
For mentally-alert Pennsylvanians with physical disabilities who need help with daily activities to remain independent.
Act 150
Personal assistance, home care aide, nursing, and durable medical equipment for individuals who meet hospital or nursing facility level of care.
CommCare
Home and community services for individuals with a medically determinable diagnosis of traumatic brain injury.
Independence
Services for persons with physical disabilities so they can live in the community and remain as independent as possible.
OBRA
Services for persons with severe developmental physical disabilities — cerebral palsy, epilepsy, and similar conditions.

How to get started

Call us or submit a referral. We'll listen, answer your first questions, and confirm which waiver may fit.
Eligibility
We help you navigate eligibility through your county Area Agency on Aging or the Office of Long-Term Living (OLTL).
Coordination begins
Once approved, we build your service plan, connect providers, and stay with you as needs change.
For discharge planners, AAAs, and providers
Submit a referral and we'll follow up the same business day. We handle paperwork, provider outreach, and eligibility coordination — and we keep referring professionals informed of the participant's status.
