Who may submit this referral

Anyone with a genuine interest in a participant's care may submit a referral: the participant, a family member or caregiver, a hospital or rehab discharge planner, a county Area Agency on Aging, a service provider, or another professional. You do not need to be enrolled in a waiver already, and you do not need permission from any agency to reach out to us.

Navigation vs. formal coordination

If you are not yet sure whether a Pennsylvania waiver applies to your situation, we can offer complimentary initial navigation — a short conversation to help you understand the landscape and identify a reasonable next step. Formal, ongoing service coordination is a separate role that applies once someone is approved for a program such as Community HealthChoices or OBRA, and is available within Alenta's confirmed 14-county service area. Submitting a referral does not enroll anyone in a waiver, does not guarantee eligibility or approval, and does not create any obligation to work with Alenta.

Alenta Service is an independent provider and is not the Pennsylvania Department of Human Services, the Independent Enrollment Broker, a county Area Agency on Aging, or a managed care organization. We do not determine eligibility, approve applications, or control waiting lists. Program rules and availability vary and may change; the responsible state or county agency makes all eligibility and enrollment decisions.

County and program availability

Formal service coordination is currently available within our 14-county area: Berks, Bucks, Carbon, Chester, Delaware, Lancaster, Lebanon, Lehigh, Luzerne, Montgomery, Northampton, Philadelphia, Schuylkill, and York counties. If you are outside this area, we may still be able to offer initial navigation or point you toward the right local resource.
