How do you qualify?

Must be a resident of Pennsylvania

Citizen of the US or Lawfully admitted immigrant

Be between the ages of 18-59 or 60-over for the AGING Waiver

Meet the financial requirements as determined by your local county assistance office ($2,100 / month or less)

Be Eligible for Medicaid/Medicare

Physical Impairment expected to last a continuous period of not less than 12 months. Including, but not limited to:

                  -Traumatic brain injury

                  -Diabetes

                  -Chronic arthritis

                  -Chronic asthma

                  -Heart or kidney problems

                  -Hypertension

                  -Cancer

                  -Stroke
