MISSION
Alenta Service Coordination LLC provides continuity of care and education, advocates for, and empowers patients and families to attain self-sufficiency. We link families with community resources and assist patients and families in navigating the various systems in order to attain the highest quality of care, while maintaining confidentiality and privacy. Our staff works daily to define the wants and needs of individuals and learn to identify possibilities and adapt plans accordingly. We consistently organize needs and best practices, and learn to navigate available opportunities while brainstorming for new possibilities. This is often the keystone of the daily work that Alenta Service coordinators practice and our experience guides us towards continued development and success. Education plays a large role in our continued development.
We educate ourselves as well as other stakeholders so that together we can grow effectively. This education process reinforces our philosophy that community relationships are the foundation of our work and effective communication of information is imperative for collaborative successes.

Our Person-Centered Approach
Alenta Service Coordination supports individuals with case management, person-centered planning, and the coordination of wrap-around services related to housing, employment, relationships, financial management, treatment & health monitoring, and recreation. With core competencies in relationship building and navigational expertise, our care coordinators are able to develop meaningful connections with government agencies, community service providers, community-based business, service organizations, and interested supporters to ensure each individual has the support needed to be successful in transitioning through each phase of their program.
